1. Service Description

The Virtual Aging in Place Home Consultation is a remote service provided via video call. It includes a live walkthrough of the client’s home environment, professional safety recommendations, and a written Home Safety Report. No physical modifications, equipment installation, or legal certifications are provided as part of this service.

2. Client Responsibilities

  • The client agrees to provide accurate and complete information via the intake form.

  • The client is responsible for ensuring a stable internet connection, a charged device (smartphone, tablet, or laptop), and the ability to show the home environment clearly during the virtual consultation.

  • The client acknowledges that recommendations are based on the information and visual walkthrough provided during the session.

3. Scope of Service

  • Recommendations are made in good faith to improve home safety and reduce fall risks.

  • This consultation does not replace an in-person home assessment conducted by licensed contractors, occupational therapists, or other certified home safety inspectors.

  • Implementation of suggested modifications is at the sole discretion and responsibility of the client.

4. Payment Policy

  • Full payment is required at the time of booking.

  • Payment secures your appointment time and consultation preparation.

  • Payments are non-refundable once the consultation has taken place.

5. Cancellation and Rescheduling

  • Cancellations made at least 24 hours before the scheduled consultation will be eligible for rescheduling without penalty.

  • No-shows or cancellations made less than 24 hours before the appointment may forfeit the consultation fee.

6. Liability Disclaimer

  • Harry Physical Therapy & Wellness is not liable for any injuries, accidents, or property damage resulting from the client’s implementation (or lack of implementation) of the safety recommendations.

  • Clients should seek additional advice from qualified professionals for major home modifications, medical equipment installation, or structural changes.

7. Privacy and Confidentiality

  • All client information, photos, and intake responses are kept confidential and used solely for the purpose of delivering this service.

  • We do not sell, share, or distribute your personal information without your explicit consent.

8. Consent

By submitting the intake form and/or booking the consultation, the client agrees to these Terms and Conditions and consents to receive emails related to the consultation and follow-up communication.
